HOSPITAL DISCHARGE PLANNING

Planning the transition from hospital to home is an important part of a hospital stay. 

Develop abilities Occupational Therapists provide in-patient consultancy upon request.

OT discharge planning can include pre-admission and post discharge assessments for a range of needs. The assessment is an opportunity to identify and respond to changes in function and health that impact on safety, mobility, and independence.

Occupational Therapists may discuss temporary or ongoing equipment, home modifications and/or suggest in-home supports if required. An assessment report is provided to your G.P and hospital to ensure you receive the very best, coordinated health care.
